Thank you for choosing Heartrise Healing and Coaching. When purchasing a program, you automatically agree that you are 18+  and acknowledge and agree to accept financial responsibility for services rendered by Holistic Practitioners/Coaches to Client.

I agree that I am legally responsible and agree to pay for all fees for services I receive, charges and expenses incurred by the Client or owed to HeartRise Healing and Coaching in connection to  Practitioners/Coaches providing services to Client.

I acknowledge and agree that I am ultimately responsible for the payment for any and all services rendered to Client.

*  Disclaimer: FEES FOR SERVICES You agree to pay for the services rendered, No REFUNDS at any time.  I also:

CONSENT FOR VIRTUAL CONSULTATIONS and Services. Internet service may not always be private or safe and risks exist about attaining information. Information will be collected when subscribing but you can always unsubscribe anytime.

  1. I understand that my Holistic Practitioners/Coaches wishes me to engage in a virtual consultation.
  2. My Holistic Practitioners/Coaches will be utilizing video conferencing technology, It will not be the same as a direct visit due to the fact that I will not be in the same room as my Holistic Practitioners/Coaches. I understand that a virtual consultation/services has potential benefits including easier access and the convenience of meeting from a location of my choosing.
  3. I understand there are potential risks to this technology, including interruptions, unauthorized access, and technical difficulties. I understand that my Holistic Practitioners/Coaches can discontinue the virtual consult/visit if it is felt that the videoconferencing connections are not adequate for the situation. By purchasing any program or consultation I acknowledge: Holistic Practitioners/Coaches are not therapy and are not Emergency Services. In the event of an emergency, I will use a phone to call 911.


HeartRise Healing and Coaching is not responsible for the delivery of any healthcare, medical advice or care.


  1. To maintain privacy, I will not share my virtual appointment link with anyone unauthorized to attend the appointment.

2.     By continuing the use of this platform,  I certify:

  • That I have read or had this form read and/or had this form explained to me.
  • That I fully understand its contents including the risks and benefits of the servce(s).
  • That I have been given ample opportunity to ask questions and that any questions have been answered to my satisfaction.

RECONOZCO Y ACEPTO QUE SOY EL ÚLTIMO RESPONSABLE DEL PAGO DE CUALQUIER Y TODOS LOS SERVICIOS PRESTADOS AL CLIENTE.

* DESCARGO DE RESPONSABILIDAD: TARIFAS POR SERVICIOS QUE USTED ACEPTA PAGAR POR LOS SERVICIOS PRESTADOS, NO HAY REEMBOLSOS EN NINGÚN MOMENTO.  YO TAMBIÉN: Entiendo Y Acepto este modo y doy mi CONSENTIMIENTO PARA CONSULTAS Y SERVICIOS VIRTUALES. EL SERVICIO DE INTERNET PUEDE NO SIEMPRE SER PRIVADO O SEGURO Y EXISTEN RIESGOS AL OBTENER INFORMACIÓN. LA INFORMACIÓN SE RECOPILARÁ AL SUSCRIBIRSE, PERO SIEMPRE PUEDE DARSE DE BAJA EN CUALQUIER MOMENTO.

MIS PRÁCTICOS/ENTRENADORES HOLÍSTICOS ESTARÁN UTILIZANDO TECNOLOGÍA DE VIDEOCONFERENCIA, NO SERÁ LO MISMO QUE UNA VISITA DIRECTA DEBIDO A QUE NO ESTARÉ EN LA MISMA SALA QUE MIS PRÁCTICOS/ENTRENADORES HOLÍSTICOS. ENTIENDO QUE UNA CONSULTA/SERVICIOS VIRTUAL TIENE BENEFICIOS POTENCIALES QUE INCLUYEN UN ACCESO MÁS FÁCIL Y LA CONVENIENCIA DE REUNIRSE DESDE UN LUGAR DE MI ELECCIÓN.

ENTIENDO QUE HAY RIESGOS POTENCIALES PARA ESTA TECNOLOGÍA, INCLUYENDO INTERRUPCIONES, ACCESO NO AUTORIZADO Y DIFICULTADES TÉCNICAS. ENTIENDO QUE MIS PRACTICANTES/ENTRENADORES HOLÍSTICOS PUEDEN INTERRUMPIR LA CONSULTA/VISITA VIRTUAL SI SIENTE QUE LAS CONEXIONES DE VIDEOCONFERENCIA NO SON ADECUADAS PARA LA SITUACIÓN. AL COMPRAR CUALQUIER PROGRAMA O CONSULTA, RECONOZCO: LOS PRACTICANTES/ENTRENADORES HOLÍSTICOS NO SON TERAPIA NI SERVICIOS DE EMERGENCIA. EN CASO DE UNA EMERGENCIA, USARÉ UN TELÉFONO PARA LLAMAR AL 911.



HEARTRISE HEALING AND COACHING NO ES RESPONSABLE DE LA ENTREGA DE NINGUNA ATENCIÓN MÉDICA, CONSEJO O ATENCIÓN MÉDICA.



PARA MANTENER LA PRIVACIDAD, NO COMPARTIRÉ EL ENLACE DE MI CITA VIRTUAL CON NADIE NO AUTORIZADO PARA ASISTIR A LA CITA.
